Job Description
Location: Portsmouth, NHShift: 8am-5PM, In-personPay: $35-$45 per Hour

Responsibilities:
- Coordinates with internal providers & external specialty providers
- Maintains inventory of medical supplies, instruments and equipment that meet quality and infection control standards.
- Analyzing patients records to identify those who are chronic disease patients and study any gaps they may have had to asses
- Communicates with patient via telephone or portal and assess patient need using standard protocols in order to coordinate appropriate care and next steps
- Help with any Cardiac specific responsibilities
- Processing documents, prescriptions refills, lab results, and telephone notes advised by the provider
- Associate degree in Nursing; Bachelor’s in nursing preferred
- Minimum of 2 years’ experience; at least one year as nurse responsible for independent decisions
(such as inpatient medical/surgical experience, triage nurse, home health nurse, clinical specialist, or other subspecialty)
- Current, unrestricted Registered Nurse licensure in Maine
- BLS required
- ACLS required for Cardiac Nurse
- Compact RN license
