Job Description
Job Title: Community Integration Worker - MHRT/C Required
Qualifications: MHRT-C with 2+ years of Human Service experience and equivalent management experience.
Pay: $22/hr
Benefits:
Full-Time employees, after completing the 60-day introductory period of employment, are offered:
Health, Vision, and Dental Insurances (100% paid by employer)
Allstate insurance (100% paid by employee)
Nine (9) paid holidays
Paid Time Off (PTO) offered after 120 days of service to use in the first anniversary year; and increases with years of service
Mileage reimbursement for ESM related services
Eligible for 401k fund after eligibility is met.
Summary: Help individuals with mental health disabilities find resources and support to lead successful lives in the community. Work collaboratively with clients and their support network to create and implement Individual Service Plans (ISP) that reflect their personal vision for independence.
Responsibilities:
Follow policies, procedures, and the organization's values, mission, and philosophy.
Maintain excellent attendance and professionalism.
Complete documentation on time and with accuracy.
Meet supervision requirements.
Manage tasks by priority.
Develop trust and respect with clients.
Be knowledgeable about local resources and funding.
Foster positive relationships and seek conflict resolution.
Contribute to a supportive team environment.
Promote independence and diversity.
